Del Professional Healthcare provides 24/7 services and support to our individuals with DD/ID and behavioral support needs through healthcare professionals who are licensed, bonded and insured. Some services we provide are:

Community Living

Our home is a perfect setting for individuals who need support in daily activities. We have great direct support employees that provide 24/7 services and support for the individual’s medical and behavioral needs through licensed services. We provide services in but not limited to meal preparation, activities of daily living(ADL’S), light housekeeping, laundry, and money management.

Behavioral Support

We have trained and experienced professionals who work with our affiliate specialists to ensure that behavior support plans are personalized to fit the unique needs of each individual and implemented to support individual behavior stability.

Skilled Nursing

A licensed nurse(LPN) is on site 24/7 to provide medical services. We also have a Registered Nurse (RN) on duty to supervise and monitor. Maintaining the individual’s health is our main priority. We also have Registered Nurse (RN) monitorying all individuals in our care. We provide daily support for the individual in areas such as: medication administration, medical appointments, pain management, care for chronic illness, insulin therapy, palliative care, G-tube feeding.

Recreation and Leisure

We provide transportation and accompaniment to all recreational activities including family visits, library, farmer’s market, shopping, van rides, religious and social events etc.

Transportation Services

We provide safe non-medical transportation to our individuals for activities such as doctor’s appointment, recreational activities, restaurants, or to any preferred place by the individual in order to maintain community connection.
